Conventional vs. Reverse Shoulder Replacements: What You Need to Know

27/04/2025

If you or someone you know is considering shoulder replacement surgery, understanding the different options available can help you make more informed decisions about your care. Today, I'd like to share some insights about two primary approaches to shoulder replacement: conventional and reverse.

The Basics: Two Approaches to Shoulder Replacement

Conventional Shoulder Replacement follows the natural anatomy of your shoulder, using a metal ball attached to a stem that fits into your upper arm bone and a plastic socket attached to your shoulder blade. This approach relies on healthy rotator cuff muscles to function properly.

Reverse Shoulder Replacement flips the natural anatomy by placing the ball component on the shoulder blade and the socket on the upper arm bone. This clever reversal allows the deltoid muscle to compensate for damaged rotator cuff muscles.

Current Trends

Interestingly, reverse shoulder replacements have become increasingly common over the past decade, now accounting for approximately 50-55% of all shoulder replacement procedures in Australia. This represents a significant shift in surgical approaches.

How Surgeons Choose
The decision between these two procedures typically depends on:
  • Age (reverse replacements are more common in older patients)
  • Rotator cuff condition (damaged rotator cuffs generally require reverse procedures)
  • Activity level and goals
  • Bone quality and overall health

Recovery Expectations

Both procedures require rehabilitation, but recovery paths and timelines can differ. Conventional replacements may allow for more natural movement but depend on rotator cuff integrity, while reverse replacements can provide better function for those with rotator cuff damage.
